Form Of Payment
All new customers may pay by check or money order and will be invoiced quarterly or annually. We will begin processing your order immediately, but must receive payment prior to activation. Once payment is received at the address below, we will contact you with password and server access information. You will receive an emailed invoice within a few hours of completing this form.

Quarterly Billing. I authorize you to bill my account for the next 3 month period. I understand that I will be charged an account set-up fee as listed on the NT and UNIX pages. In the future, I agree to be charged every 3 months I stay with Fusion Source Web Hosting Services. I understand that I will receive a quarterly invoice and nonpayment will result in loss of service. Annual Billing. I authorize you to bill my account for the next 1 year period. I understand that the regular set-up fee will be waived. In the future, I agree to be charged every 12 months I stay with Fusion Source Web Hosting Services. I understand that I will receive an annual invoice and nonpayment will result in loss of service.
